Enrollment Step 2

Now that you're an HSLDA member, you're ready to fill out our official enrollment form. Please complete the steps below to provide us with the necessary information for your homeschool records.

  • Fill out the enrollment form with your family's information.
  • Provide details for each student you are enrolling.
  • Review all information for accuracy before submitting.

Enrollment Step 3

Your final step to becoming a verified member of Isolele Academy is to pay your enrollment fee. A one-time payment of $40.00 covers your family's enrollment for the year.

Church School Enrollment & Withdrawal Form

Please complete this form in full. Parents should submit this completed form to their child’s previous school as an official withdrawal and records request notice. All records will be released to the parent/guardian, who is serving as the home-based instructor under Isolele Academy’s church school covering.

Church School Enrollment Statement

By signing the enrollment form, I acknowledge that I am enrolling the above-named student into Isolele Academy, a church school operating in compliance with Alabama Code Section 16-28-1. I understand that I, as the parent or guardian, am responsible for providing education, record keeping, and oversight for this student.

Withdrawal & Records Request Notification

Please accept this document as formal notice that the student listed above has withdrawn from your school and is now enrolled at Isolele Academy, a church school operating in accordance with Alabama Code Section 16-28-1.

We respectfully request that all cumulative educational records, including attendance, grades, immunization records, special education plans (if applicable), and any other relevant files, be released to the parent/guardian at the mailing address provided above.

This notice is submitted to fulfill the parent/guardian’s legal requirement to notify the local school of withdrawal and enrollment into a church school.
